TATTOO CONSENT FORM

I am over 18 years of age and accept full responsibility for the administration and willing participation, in the actor receiving a tattoo by "Yaya" crew.
I release and give any claims and rights I may have against "Yaya" from all manners of liabilities, claims,actions and demands in law in inequity, which I or myheirs have or might have now or hereafter by reason of complying with my request to be tattooed. I understand that I will only be tattooed with a new needle, and that all the appropriate instruments will be sterile. I acknowledge that I have freely and voluntarily chosen the type of tattoo received, I have approved location and size of the design prior to being tattooed.
I also understand that though the tattoo is professionally performed, there is always risk such us, but not limited to infection occurring of the possibility of human error.
IMPORTANT 
I do not to my acknowledge, now or previously suffered from any of the following diseases: Diabetes,Hepatitis,Acquired Immune Deficiency (A.I.D.S.), venereal disease of any kind, haemophilia, epilepsy, or any transmittable diseases (or relevant skin disorders).
I have read all the terms and conditions contained in the above and fully understand and agree to be bound by all the terms contained herein. I do hereby certify and swear that all the representation made by me are true and that I am not under the influence of alcohol or drugs.
